Community based support is all about you

No matter what your interests, St.Giles can tailor a home support, social and recreation, and leisure program to suit your needs. Our community based support service can provide opportunities to become more independent in your own home, engage in social activities, and cultural events such as art, music, and sport, or to develop new skills or experiences that are meaningful to you.

Our Support Professionals will support you to get connected, be connected and stay connected with people, services and events to meet your social and community participation goals. These goals can consist of:

  • Being part of the community you live in
  • Engaging in further education
  • Gaining work readiness skills or experience
  • Developing skills to increase your independence
  • Catching public transport
  • Budgeting and shopping

Your St.Giles options can be provided on an individual basis or as part of a group.

We can support you to find other people who share common interests and goals to help maximise your experience in activities that are meaningful to you.

Community Based Support is all about you. At St.Giles, we want to help you determine what it is you want to achieve, how you want to achieve it, and who you want to achieve it with.

Get organised

You may have a number of different appointments with doctors and specialists – so start a notebook of all conversations and appointments. Keep all documents together in a single folder – you will need to keep receipts, invoices, forms etc…

Review funding options

Every case is unique. StGiles can help you identify the services to meet your needs and the best funding option available for your situation.

Funding options include:

  • Medicare plans
  • Private health
  • NDIS
  • Self-funded: you can pay the full fee without any funding.

NDIS Support Coordination

If you are eligible for the NDIS, it may take several months for your funding and Plan to be approved. StGiles can assist you with NDIS plan co-ordination and management (including some or all of the funding including financial, legal and administrative requirements).

NDIS service provider

Once you have an approved NDIS Plan, it is up to you to choose a Disability Service Provider(s). StGiles offers an extensive range of services for both adults and children living with disability. Our services can be tailored to meeting individual needs, delivered individually or as a package, and can change as your needs evolve.
